Weeks 2 to 8: The Shedding Nobody Warns You About Properly

Androgenetic alopecia is a genetic disorder, and it is progressive. It may not affect the transplanted follicles, but it influences the surrounding native follicles. In weeks 2 to 8, men also lose hair around the transplanted area, not just from it. This is recipient-site effluvium (shock loss), and a 2026 review of follicular unit excision complications reported an incidence of 0.15% to 15%, occurring 2 to 8 weeks after surgery, with regrowth expected around month 3. That review lists topical minoxidil among the supportive measures used during this period.

The second reason is the medication rather than the surgery. Minoxidil alters the hair cycle by shortening the resting phase and prolonging the growth phase. As older hairs are shed before new growth begins, this can temporarily result in increased shedding.  

Doctor's note: Two shedding events overlapping in the same six weeks is why month 2 feels worse than month 0. Both resolve. Neither means the plan failed.

Month 3: The First Signal Worth Trusting

Why most people never get this far

One of the main reasons some people get better results from minoxidil than others is consistency. One study demonstrated this gap and reported that out of 400 patients, 86.3% stopped using the treatment and only 13.7% stayed on treatment. Another factor that can influence treatment response is an enzyme called sulfotransferase. Minoxidil gets converted to its active form, minoxidil sulfate, by an enzyme called sulfotransferase, which is present in hair follicles. People with low levels of this enzyme respond slowly and may not get the same results, as the drug fails to convert into its active form. Minoxidil may be ineffective in 40-50% of patients. 

What Six Months Does Not Promise

Six months of consistency can deliver great results, but it cannot deliver miraculous transformations. Grafted hair is yours for good. The hair you were born with follows its own trajectory, and pattern hair loss keeps moving around the transplanted hair.
